**PUBLIC NOTICE**

NOTICE:

The Georgia Firefighter Standards and Training Council voted to revise the "Physician's Affidavit" requirement for state certification.

Effective December 1, 2005, the "Physician's Affidavit" portion of the certification package is changed to "Medical Affidavit". The required medical examination may be signed by a physician, physician assistant, or a nurse operating under a physician's authority.

New Form - Change of Status Form
The Change of Status Form is to be used where fire department members are:

• Placed on MEDICAL LEAVE for over 1 month.
• Returned from MEDICAL LEAVE.
• Placed on MILITARY LEAVE for over 1 month.
• Returned from MILITARY LEAVE.
• APPOINTED/HIRED
• TERMINATED or left the fire department
